    Here is their website page that will help you obtain the record. You can wither download a form to mail or you can order online, but that means going trough vitalcheck.com and they charge a hefty fee. Your best bet is the first option.

    You can also try the Mormon’s website: http://www.familysearch.com

    I have found it very helpful in searching for my family geneology. You certainly don’t have to be a mormon or have mormons in your family tree to access it, as they keep careful records of anybody and everybody, and others submit their own records to the website. Good luck!
